Percutaneous Endoscopic Lumbar Discectomy for Unilateral Paramedian and Foraminal Disc Herniation

Abstract
We report the efficacy of percutaneous endoscopic lumbar discectomy for the patient with intractable unilateral leg pain in both paramedian and foraminal disc herniation. A 43-year-old man with severe left buttock, left anterior thigh, and left lateral calf pain that had increased over two months was presented. An magnetic resonance imaging revealed both paramedian and foraminal disc herniation at L4-5 level on the left. A computed tomography scan showed unilateral paramedian and foraminal disc herniation at L4-5 on the left. Considering soft lumbar disc herniation and the patient's need for minimal invasive surgery, we decided to perform percutaneous endoscopic lumbar discectomy. Herniated disc fragments were completely removed and confirmed with a magnetic resonance imaging, and previous symptoms were much improved. The perioperative Visual Analogue Scale score for back and leg pain was improved and the patient showed satisfactory outcome on the Oswestry Disability Index. In the surgical treatment of unilateral paramedian and foraminal disc herniation in young and middle-aged patients, percutaneous endoscopic lumbar discectomy can be a safe and effective alternative surgical option instead of open lumbar surgery.
